ImmediCare handles PHI only as a HIPAA Business Associate and only under a signed Business Associate Agreement. PHI is never used for marketing purposes and is handled in accordance with our HIPAA Compliance Statement.

We may share information with trusted service providers who assist in our operations (including Google Analytics for website analytics) under confidentiality obligations. We do not share personal information with third parties for their own marketing purposes.

Data retention

We retain form submission data (name, email, phone, practice name) for up to 24 months from the date of submission, or until you request deletion, whichever is sooner. Server log data is retained for up to 90 days. PHI handled under a Business Associate Agreement is retained and disposed of in accordance with the terms of that agreement and applicable HIPAA requirements.

Your privacy rights (CCPA / US state laws)

Depending on your state of residence, you may have the following rights regarding your personal information:

  • Right to know: You may request information about the personal data we have collected about you and how it is used.
  • Right to delete: You may request that we delete personal information we have collected from you, subject to certain exceptions.
  • Right to opt out: We do not sell personal information. If this changes, you will have the right to opt out.
  • Right to non-discrimination: We will not discriminate against you for exercising any of your privacy rights.
